Central AL Scholarship Application to Attend the Summit

Purpose: To recognize recipients for their contribution and desire to engage within the Central AL chapter and HPNA as a professional society home.

Eligibility: Be a professional nurse (RN, LPN, Nurse Aid, Nurse Practitioner) in the states of AL, GA, MS, TN; Be able to attend the University of Alabama at Birmingham (UAB) Center for Palliative and Supportive Care 2024 Forging the Future of Palliative Care Summit (Summit) November 14th – 16th, 2024 at the UAB Hilton; Sit for an interview about the benefit of HPNA membership; Give a presentation at a chapter event in the 2025 – 2026 year.

Award: HPNA state and national membership paid for 1 year; Attendance and hotel costs (at the Homewood Suites, Birmingham 2 nights MAX) for the 2024 Summit.

Requirement: Provide a 750 word (or less) essay describing your desire to attend the Summit and how it will advance your career goals in hospice and/or palliative care and describe how you plan to engage with the Central AL HPNA chapter in a sustained manner over the next 24 months.

Number of Awards to be given: 2 awards will be given (eligible – nurse assistants, licensed practical nurses, registered nurses, nurse practitioners)

Timeline: 1. Deadline for submission:9/16/2024; 2. Notification of Award: 9/27/2024; 3. Confirmation of attendance at the Summit and finalization of travel plans:10/03/2024.

All applications will be evaluated by Central AL HPNA Chapter Executive Board members and selected members based on communicated growth and benefit for attending the Summit, commitment to serve the chapter, and clarity of writing.
